For each question, Â rate your agreement on a scale from 1 (Never) to 5 (Always). Tally your score and check your results with the scale below.
Codependency Self-Assessment Questionnaire:
- I find it difficult to make decisions without seeking others' approval first.
- I often put others' needs before my own, even when it's detrimental to my well-being.
- I feel responsible for other people's feelings or actions.
- I have trouble identifying what I'm feeling in the moment.
- I struggle to say "no" when others ask for my help or time.
- I feel anxious or uncomfortable when I'm not in a relationship.
- I tend to stay in relationships even when they're unhealthy or abusive.
- I often try to "fix" other people's problems without being asked.
- My mood depends heavily on how others treat me or react to me.
- I have difficulty setting and maintaining personal boundaries.
- I often feel guilty when I do something for myself.
- I find myself making excuses for my partner's or family member's negative behaviors.
- I avoid conflict at all costs, even if it means suppressing my own needs or opinions.
- I base my self-worth on others' approval or perception of me.
- I find it hard to relax or have fun without worrying about others.
- I often feel responsible for the success or failure of my relationships.
- I struggle with expressing my true feelings to others.
- I frequently feel resentful or angry, but have trouble expressing these emotions directly.
- I tend to attract partners or friends who need "rescuing" or have significant personal problems.
- I often feel empty or lost when I'm not actively caring for someone else.
Scoring:
- 20-40: Low likelihood of codependent tendencies
- 41-60: Moderate codependent tendencies
- 61-80: High codependent tendencies
- 81-100: Very high codependent tendencies
